Nearly 21 million Americans will receive a $90 health insurance subsidy without having to apply for automatic payment.
The U.S. Centers for Medicare and Medicaid Services (CMS) announced that approximately 20.8 million Americans enrolled in the "Original Medicare" Part B plan are eligible for the subsidy. For most beneficiaries who have opened direct deposit services, payments are expected to arrive around October 8, while paper checks will arrive in late October. It is worth noting that no application is required for this subsidy.
This payment is intended to offset a portion of Medicare Part B premiums for October 2026. Currently, the standard monthly premium is $202.90, which means this subsidy covers approximately 44% of the standard monthly premium amount.
Who qualifies for this $90 Medicare subsidy?
The threshold for eligibility is not just as simple as registering for Medicare. Under CMS rules, recipients must meet the following conditions: participate in the Original Medicare Part B plan, live in the United States, do not assist in paying Part B premiums through Medicaid, and do not need to pay an Income-Based Monthly Adjustment Amount (IRMAA).
This rule excludes four major groups from the plan: members participating in Medicare Advantage, beneficiaries who need to pay IRMAA due to high income, people receiving Part B premium assistance through Medicaid or Medicare savings plans, and beneficiaries living outside the United States.
Among them, the exclusion scope for Medicare Advantage participants is particularly broad. Although many insured people still need to pay Part B premiums, about 35 million people participate in the private Medicare Advantage plan. At the same time, the Medicare Advantage plan is also preparing for pricing and network changes in 2027.
Payment method: Fully automated processing
Eligible beneficiaries do not need to register or submit claims. Most payments will be made through the same direct deposit channel used by the social protection Administration (SSA). Users who do not have a direct deposit service will receive a Treasury Department check sent to their Med registered address with the note stating "Medicare Improvement Fund Payment".
If you have any questions about qualification recognition, you can call the 1-800-MEDICARE hotline for consultation; if you have questions about payment status, you can contact the social protection bureau from October 15.
The subsidy follows a $500 rebate to some Affordable Care Act (ACA) enrollees.
The $90 subsidy could quickly be absorbed by rising costs
This amount comes from the Medicare Improvement Fund, which currently has a balance of just over $2 billion. The Kaiser Family Foundation (KFF) estimates that the subsidy project will consume most of the fund.
The bigger problem lies in the subsequent impact. KFF estimates that Standard Part B premiums could rise by about $79 a year by 2027. This means that the vast majority of the one-time $90 subsidy is likely to be completely offset in the face of higher premiums next year. In addition, other fees such as Part D premiums, deductibles and Medigap supplemental insurance coverage may also increase.
These fee increases will also interact with the upcoming social protection cost-of-living adjustment (COLA), which determines the increase in monthly retirement benefits next year.
